Iron Studies Explained: Ferritin, Iron, TIBC & Saturation

Iron panels include ferritin (stores), serum iron, TIBC/transferrin, and transferrin saturation (iron ÷ TIBC). Use your lab's ranges.
Patterns
- Iron-deficiency: low ferritin + low saturation; TIBC often high; CBC may show microcytosis.
- Inflammation pattern: ferritin normal/high with low iron and low/normal TIBC; saturation low.
- Overload: very high ferritin with high saturation – consider further testing.
Educational next steps
- Look for loss (menstrual/GI), low intake, or demand (pregnancy).
- Diet: iron-rich foods + vitamin C; avoid tea/coffee with iron-rich meals.
- Oral iron education (not a prescription): elemental 40–65 mg once daily or every other day; constipation common.
- Repeat ferritin/hemoglobin often in 4–8 weeks to confirm response.
